To provide dental care, we collect personal health information such as your name, contact details, health history, dental and medical records, treatment notes, X-rays and images, and insurance and billing information.

We use administrative, physical, and technical safeguards to protect your personal health information against loss, theft, and unauthorized access, use, or disclosure.
Under PHIPA, you have the right to access your records and to request corrections. Contact our office to make a request.
We retain records for the period required by law and applicable regulatory (Royal College of Dental Surgeons of Ontario) requirements, then securely dispose of them.
